Let us handle the entire insurance process.

If we’re in network with your plan, you won’t have to submit any insurance paperwork yourself, unlike at many other private practices. Our practice manager, Anna, handles the billing and answers your questions. If we’re out of network with your plan, we’ll give you a superbill, an itemized receipt you submit to your insurance company for possible reimbursement, and Anna can help you understand what your plan may cover.

If you have questions regarding your insurance coverage, including the costs associated with deductibles and copayments, please contact Anna at 773-414-4577 or email Anna@pac.clinic. Please be sure to include the name and date of birth of the policy holder, the individual ID number, group ID number, and the phone number for the insurance carrier.

Treatment guidelines, including specific details regarding the billing process can be found using the links below. In order to use the time efficiently, clients are encouraged to review and fill out the following paperwork prior to the initial session. You can print these forms or email them to your therapist. We’ll be happy to answer any questions regarding the material at the beginning of the session.

Insurance Carriers:

Most of our therapists are in-network with:
  • BCBS PPO
  • BlueChoice PPO
  • BlueChoice Select
Additionally, some of our providers are also in-network with:
  • UBH
  • Optum
  • UMR
  • UnitedHealthcare
  • UnitedHealthcare Surest
  • UnitedHealthcare StudentResources

These plans cover online therapy by secure video as well as in-person sessions.

Although we are out-of-network with all other carriers, PAC will gladly provide you with a superbill that you can submit to your insurance carrier. Please contact Anna Wlodarczyk our Practice Manager, Anna@pac.clinic to determine your coverage.

Out-of-Pocket Clients:

If you do not have insurance or out-of-network coverage, the standard fee is $125 – 150 per session, depending upon the clinician.
